Senior Physical Therapist - Home Health Care Specialist

Position Overview

As a Home Health Physical Therapist, you'll provide personalized therapeutic care to patients in their homes, working collaboratively with our multidisciplinary healthcare team to achieve optimal patient outcomes. Core Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions aligned with physician-directed care plans
• Create and implement home exercise programs, ensuring patient/caregiver understanding
• Document patient progress and maintain detailed clinical records
• Collaborate with healthcare team members to optimize patient outcomes
• Manage caseload efficiently while maintaining high-quality care standards
• Train patients and caregivers in the proper use of therapeutic equipment and devices
• Participate in care plan reviews and team meetings

Required Qualifications

• Current Physical Therapy license in good standing
• Bachelor's degree in Physical Therapy (DPT preferred)
• BCLS certification (or ability to obtain within 30 days)
• Valid driver's license and reliable transportation
• Excellent communication and interpersonal skills
• Strong organizational and time management abilities
